Sunken Eyes and Tear Trough Hollows: Causes and Non-Surgical Options

Juvelook addresses the tear trough groove, and Rejuran and PRF address thin, rough under-eye skin. If under-eye fat bulges severely and the lower eyelid skin has loosened as well, surgery is usually the route rather than injections.

In three lines

A hollow look comes from a tear trough and lost upper-cheek volume, bulging under-eye fat, or thin, rough skin.

Whether injections or surgery fit depends on how much the fat bulges and whether the lower lid skin has loosened.

Under-eye skin is thin, so a nodule is easy to see.

Why do sunken eyes and a tear trough look hollow?

One pattern is a groove from the inner corner down toward the pupil, together with lost upper-cheek volume. Another is fat that has come forward, so the area below looks more hollow. A third is thin skin, where fine lines and a rough surface show first. With age, lower eyelid skin grows thinner and loses firmness.

One research group sorted 78 people with a tear trough into 3 groups by how much the under-eye fat bulged and whether the lower eyelid skin had loosened, and used surgery only for the group with a severe fat bulge and loose skin.

Cellinique offers Rejuran, Juvelook, and PRF for the under-eye area.

What does Rejuran do for under-eye skin?

Rejuran is an injection of PN (polynucleotide). How eye-area texture changed is covered in the Rejuran eye-area texture article. If fine lines are the main concern, read the eye-area wrinkle article alongside it.

What Rejuran addresses is thin, rough texture rather than the depth of the groove. In a Korean study that injected PN, the ingredient in Rejuran, around the eyes and compared it with non-crosslinked hyaluronic acid on the other side, overall improvement scores were similar, and the improvement rates for firmness, moisture, roughness, and pore volume were higher on the PN side.

How is Juvelook used on a tear trough?

Juvelook is an injectable treatment combining collagen-stimulating PDLLA particles with hyaluronic acid.

In a small case series of Korean women, Juvelook was injected in small divided amounts, and assessments through 6 months rated the tear trough grade as improved. People whose mid-cheek was hollow received another injection in the cheek first.

What about PRF for fine lines and under-eye texture?

PRF is a treatment that separates fibrin from your own blood, and the steps from the blood draw through application are set out in the PRF guide.

In a review of blood-derived injections around the eyes, PRF was linked to improvement in texture, fine lines, and crepiness. Reactions were mild and short-lived. The change had often grown smaller by about 6 months.

What reactions remain after under-eye treatment?

The material and the number of sessions differ by treatment, so the reactions and the cautions differ too.

※ Under-eye skin is thin. A nodule or a change you can feel shows easily. There is a case of firm nodules on both sides, without redness, 3 weeks after a larger-particle product in the Juvelook line was injected into the tear trough. Nodules and lumps are also reported with polylactic acid products. Whether a lump is an injection-site bump or a nodule is explained in the injection-site bumps and nodules article, and how a nodule is handled is in the Juvelook nodule guide. The Juvelook tear trough study told people not to rub at home. Rarely, infection or a blocked-vessel complication is also a risk considered with under-eye injections.

What affects the cost of under-eye treatment?

The cost varies with the treatment, the number of sessions, under-eye only or the whole eye area, and whether another treatment is added; the cost of each treatment is listed on the pricing page.

What to write down before booking under-eye treatment

Note whether morning swelling changes the appearance of the hollows, and allow recovery time before events or photo shoots.

  • □ Whether under-eye hollows look different in the morning and evening because of swelling
  • □ Any bulging spot under the eye
  • □ Dates of past under-eye filler or other treatment
  • □ Event or photo dates (whether a bruise overlaps)
  • □ Medicines or supplements that thin the blood

Frequently asked questions

Q1. What about bruising and swelling after an under-eye injection?

In some cases in the Juvelook tear trough study, mild bruising and swelling settled within 48 hours. That study started after numbing cream was applied and left on for 15–20 minutes.

Q2. How many sessions are needed?

The Juvelook tear trough case series was mostly 1 session, and the study of PN, the ingredient in Rejuran, was 3 sessions spaced 2 weeks apart. At the visit, this number is adjusted to the condition of the skin.

Q3. How long do the changes last?

In the Juvelook case series, the change continued through the 6-month check. In the PN study, the gains in firmness and moisture grew smaller over time, and for PRF the improvement had often grown smaller by about 6 months.

Q4. Can an injection help if under-eye fat is bulging?

An injection does not remove the bulging fat; it addresses the groove underneath and the skin texture. If the bulge is mild and the groove below stands out more, Juvelook is the match, and if the fat bulge is severe and the lower eyelid skin has also loosened, plastic surgery and oculoplastic care assess a surgical approach.

Q5. Will the color of dark circles fade as well?

A shadow from a groove and a dark color from pigment or blood vessels come from different causes. Rejuran, Juvelook, and PRF address the groove and skin texture, and a dark color needs its own cause-based approach.
